Dear Parents,

We are in the process of filling the on-line CBSE registration forms for class IX and XI. We request you to verify the details in the form that is being forwarded to you. The details in the form have been filled as per the school records. If there is any change involved in details of Date of birth, Fathers Name, Mothers name & spelling of the students Name, please write it in the corresponding box on the right hand side (please use CAPITALS only). Any change in details should be accompanied by an affidavit duly signed by a first class magistrate.

Please tick the relevant boxes. All fields are mandatory. We request both parents to sign the form.

You are requested to send the verified form, affidavit ( if any), two black & white passport size photographs clearly indicating the students name and date photograph was taken (on or after 1.04.2011) and cash Rs. 100/- as Board registration fee by Wednesday 24th August, 2011 to the class teacher.
